一、本申报书填写内容必须实事求是,不得弄虚作假. I. This application must be filled out based on facts. No fraud is allowed.

二、本申报书根据《关于实施领军型创新创业团队引进培育计划的意见》(浙委办发〔2014〕11号),引进海内外领军人才和团队来浙创新创业,根据《浙江省领军型创新创业引进培育计划实施细则(试行)》(浙科发政〔2014〕107号)等文件制定,填表前请认真阅读上述文件. II. This form is in accordance with Proposal about Implementation of Leading Innovative and Entrepreneur Team Introduction Program (ZWBF [2014] No.11), and it is prepared according to Rules for the Implementation of Leading Innovative and Entrepreneur Team Introduction Program of Zhejiang (Pilot) (ZKFZ [2014] No.107) and other documents so as to introduce domestic and overseas leading talents and teams to Zhejiang for entrepreneurship;

and please read the documents above carefully before filling out the form.

三、浙江省引进领军型创新创业团队实行网络申报,不接受纸质申报.具体申报流程为:(1)登录浙江省创新人才团队信息管理系统(cxrc.zjkjt.gov.cn),在线获得申报账号和密码;

(2)申报团队在线填写申报书及提交申报附件;

(3)用人单位在线审核;

(4)企业所在地科技行政管理部门在线审核,包括县(市、区)科技局和市科技局(委);

(5)申报团队下载申报书,签章;

(6)县(市、区)、市科技部门签章推荐,提交纸质申报材料. III. The application is only available on-line and no paper document is accepted. On-line application procedure is: (1) applicants obtain the application account and password on-line(cxrc.zjkjt.gov.cn);

(2) submit application form and attachments on-line;

(3) employer on-line review the application materials;

(4) designated administrative departments (the City and Municipal Science &

Technology Department) conduct the on-line review;

(5) applicants download, sign and stamp application form and attachments;

(6) the designated administrative departments sign and submit the paper documents .

四、申报自2014年7月25日开始,至2014年8月31日截止,申报资料提交后,不得修改. IV. The application opens on Jul

25 2014, closes on Aug

31 2014;

application materials cannot be modified after being submitted.

五、申报团队及用人单位请按照申报公告及申报指南相关时间进度要求,合理安排各环节申报时间,逾期不予受理,责任自负. V. The applications and the employer shall arrange the application reasonably according to the application announcement and schedule;
