1 January 2018, please state "2018" in the field of "Effective Year". (3) 如你现有的登记证是於2017年12月31日届满,你必须於2017年11月30日或之前作出选择.否则,你下一张由2018年1月1日开始生效的登记证仍然是一张1年有效期的登记证,而你的选择要待再下一张由2019年1月1日开始生效的登记证才生效.如你仍欲就2018年1月1日开始生效的登记证作出逾期选择,请说明原因〔例如:不在香港、疾病或其他合理因由〕导致你未能准时作出选择.If your current certificate expires on

31 December 2017, your election must be made on or before

30 November 2017. Otherwise, your next certificate commencing on

1 January

2018 would still be a 1-year certificate and your election would only be applicable to the certificate commencing on or after

1 January 2019. If you wish to pursue your late election for the certificate commencing on

1 January 2018, please state the reason (for example, absence from Hong Kong, sickness or other reasonable cause) preventing you from lodging the election on time. (

4 ) 如签署人是「法团董事」或秘书公司,必须填上其商业登记号码.如未能提供商业登记号码,请提交该公司成立为法团的地方的政府发出的「公司注册证书」.否则,请填上签署人的身分证号码或护照号码.If the signer is a "corporate director" or secretarial firm, please fill in its business registration number. If business registration number is not available, supply a copy of the Certificate of Registration of the company issued by the relevant government authority in its place of incorporation. In all other cases, please fill in the identity card number or passport number of the signer. (5) 尽管总行或分行业务在三年登记证有效期内结束营业,已缴的商业或分行登记费及徵费均不会获得退还.The business registration fee and levy paid will not be refunded even though the business or the branch may cease to be carried on before the expiry of the validity period of a 3-year certificate.
